Sickness self-certification form should be completed for sickness absence for up to 7 calendar days. These self-certificate forms can be collected from reception or via the link below.

Under normal circumstances sickness absence of more than 7 calendar days requires a medical certificate from a doctor. This can be obtained by making a routine GP appointment. Should you need a further sick note please contact reception as the GP may want to see you again.
