New Patient Registration
Applying to join the surgery
If you wish to become a patient at Bootle Village Surgery please ask at reception if you are in the catchment area for the surgery (see below). If you are you will be given a new patient pack and need to book a new patient medical appointment with our Healthcare Assistant. Anyone under the age of 16 does not need a new patient medical with the Practice Nurse. Alternatively, please refer to the information on How to register with a GP practice and to download a GMS1 in the link HERE.
NEW PATIENT INFORMATION
This Practice DOES NOT prescribe the following drugs:
We ask that you complete the paperwork in the new patient pack.
You do not have to provide ID when registering with a GP but it is helpful to do so. It would be helpful if you can provide at least 1 of the documents below when registering with a GP;
- Birth Certificate
- HC2 Certificate
- Rough sleeper identity badge
- Hostel registration mail forwarding letter
The Practice has a broad catchment area and we can accept new patients from L20, L21, L22 and L23.
The exception to this rule is if a member of your household is already a registered patient with the surgery. Please see reception if you do not live within this catchment area as special requests can be made to the Practice Manager.
