New Patient Registration
Applying to join the surgery
If you wish to become a patient at Bootle Village Surgery please ask at reception if you are in the catchment area for the surgery (see below). Alternatively, please refer to the information on How to register with a GP practice and to download a GMS1 in the link HERE.
NEW PATIENT INFORMATION
This Practice DOES NOT prescribe the following drugs:
Zopiclone
MST
Oxycodone
Tramadol
Buprenorphine
Pregablin
Diazepam
Nitrazepam
Temazepam
Fentanyl
We ask that you complete the paperwork in the new patient pack.
You do not have to provide ID when registering with a GP but it is helpful to do so. It would be helpful if you can provide at least 1 of the documents below when registering with a GP;
- Passport
- Birth Certificate
- HC2 Certificate
- Rough sleeper identity badge
- Hostel registration mail forwarding letter
The Practice has a broad catchment area and we can accept new patients from L20, L21, L22 and L23.
The exception to this rule is if a member of your household is already a registered patient with the surgery. Please see reception if you do not live within this catchment area as special requests can be made to the Practice Manager.
