Applying to join the surgery
If you wish to become a patient at Bootle Village Surgery please ask at reception if you are in the catchment area for the surgery (see below). If you are you will be given a new patient pack and need to book a new patient medical appointment with our Healthcare Assistant. Anyone under the age of 16 does not need a new patient medical with the Practice Nurse.
We ask that you complete the paperwork in the new patient pack before attending your appointment and also ask that you bring identification with you such as photographic ID; passport, driving license and anything with your name and address on. These will be photocopied and returned to you at your appointment. Please inform the surgery as soon as possible if you are not able to provide these.
The Practice has a broad catchment area and we can accept new patients from L20, L21, L22 and L23.
The exception to this rule is if a member of your household is already a registered patient with the surgery. Please see reception if you do not live within this catchment area as special requests can be made to the Practice Manager.
